Service Repair & Calibration RMA Information Form

Please provide the following information to receive your RMA (returned material authorization).

Radian offers fixed pricing on all service & calibrations. An RMA number will only be issued after a properly authorized purchase order is submitted.

To ensure that the correct information is entered into our RMA system, please provide the following information.

  1. Product model and serial numbers:
  2. Reason for return with any failure modes and/or error codes if any:
  3. Your company's current billing address (the company to be billed - please provide, even if under warranty):
  4. Your current shipping address (where the unit is to be returned). If same as billing address leave blank:

  6. Preferred shipping method (If you are requesting that we use your shipping account number, please provide.):
  7. Please specify whether you will be using a purchase order (placed at the time of sending units in will avoid delays in turn around time) or credit card:

This information is required prior to issuing any RMA's.
